 1. State Group Life Insurance Increases Effective April 1, 2009

All eligible employees under the Fort Dearborn state group life insurance plan (formerly Medical Life Insurance Company) whose yearly gross compensation rate increased on or before April 1, 2009 had an increase, if applicable, in their insurance coverage amount and payroll deduction premium.

The increases in coverage and payroll deduction premiums are in accordance with the wage/coverage schedule of life insurance: http://www.osc.state.ct.us/2009memos/numbered/200905.htm.

Payroll deduction premium changes for increased life insurance coverage were reflected in paychecks dated April 9, 2009.

If you have any questions, contact a representative from the Human Resources Benefits Unit at benefits@uconn.edu or (860) 486-0400.

On April 3, 2009 the revised Form I-9 (Rev. 02/02/09) went into effect. Please be aware that you may no longer use a previous version of the form. The revised form provides greater security and effectiveness by no longer allowing the use of expired documents for identification purposes. The USCIS has also updated their "Handbook for Employers - Instructions for Completing Form I-9" to reflect the revised requirements.

The revised form can be found at the following link http://www.uscis.gov/files/form/I-9_IFR_02-02-09.pdf and the revised handbook can be found at http://www.uscis.gov/files/nativedocuments/m-274.pdf

If you have any questions regarding the form I-9 please contact Ellen Lowe in the Payroll Department at 486-6915, or ellen.lowe@uconn.edu.
