LIFE CHANGES AFFECTING MEDICAL/DENTAL INSURANCE
It is the employee's responsibility to notify
the Department of Human Resources
of any life changes that will effect their
health insurance coverage.

| Birth or Adoption | Within 30 days from date of birth or adoption | Dependent added the first of the month following the date of birth or date of adoption |
| Divorce or Legal Separation | Within 30 days from date of divorce/legal separation | Dropped the first day of the month after the event (COBRA offered up to 36 months) |
| Employee Death (covering dependents) | "Employment Separation Form" forwarded by the department head. | Dependents dropped the first day of the month after date of death. (offered COBRA for up to 36 months) |
| Family Entering USA | Within 30 days of entering USA | Added on the first of the month after the date of entry |
| Loss of Coverage | Within 30 days of last date of coverage | Added on the first of the month after the date of loss of coverage |
| Marriage | Within 30 days from date of marriage | Spouse added the first of the month following the event |
| Domestic Partner | Within 30 days from submitting affidavit | Partner added the first of the month following submission of affidavit and approval from State Comptroller's Office |
| Civil Union | Within 30 days from date of civil union | Partner added the first of the month following |
| Graduating Dependent | Within 30 days from date of graduation | Dropped first day of the month after the date of graduation (offered COBRA for 36 months) |
| Overage Dependent - Dental | At 19th birthday | Dropped the first day of the month following the 19th birthday (offered COBRA for 36 months) |
| Overage Dependent - Medical | At 19th birthday. If full time student, see note below | *Dropped the first day of the month following the 19th birthday (offered COBRA for 36 months) |
Please note:
If the dependent
is under age 23 and a full time student,
the employee must supply
the Department
of Human Resources with documentation each semester, in order for coverage to
continue.
The documentation should be from
the dependents academic institution (registrar's office),
and verify the dependent's full-time
student status.
