
Procedure for Reporting Work Related Injury for Classified Employees

The steps for reporting a work-related injury are as follows:
- Report injury to your supervisor at the time of the injury.
- Supervisor should report injury to our managed care company at 1-800-828-2717. Complete a First Report of Occupational Injury and forward it to Human Resources at Unit 5075. New forms are available by contacting Linda Dann at (860) 486-2432.
- If medical attention is necessary, it must be through the State of Connecticut approved network of providers. Directories of the providers are available through your department secretary, or at Human Resources.
- Employees are required to notify Human Resources AT ONCE when time is lost due to a work-related injury or illness. Upon notification, both Human Resources and Payroll will send forms which must be completed and returned before any payment of workers' compensation moneys can be made. If the forms are not returned immediately, a delay in workers' compensation payments will result. Failure to return the sick leave election form to Payroll will prevent the employee from using accrued time while waiting for workers' compensation moneys and in any use of accruals to make a full paycheck. No payment will be made by worker's compensation prior to receipt of the filing status form.
