Sick Leave Policy for Faculty

TO: Deans, Directors, and Department Heads

In order to assure greater accountability during external audits, the Provost requests that the following information be included when department heads wish him to authorize sick leave with pay for a member of the faculty:  

  1. the beginning date and an anticipated end date sick leave.
     
  2. a physician's statement describing the nature of the disability the nature of the disability and stating the expected date for the faculty member's return to work.

In the event the faculty member is unable to return to work as expected the department head may request an extension. Each request for an extension should also be accompanied by a statement from the faculty member's physician.
